Your First Visit To Our Naperville Dentist Office

In preparation for your first visit to Gentle Family Dentistry of Naperville, you will be asked to complete a registration form and a health history document.

Please bring:

• A government issued photo ID
• Insurance information
• Your current medication list

We ask that you have your previous dental x-rays emailed to our office. If you are not able to obtain your previous x-rays, new ones will be taken. This is part of our office policy.

During your new patient visit, your dental and medical history will be reviewed. Your new dentist will do a comprehensive exam and oral cancer screening. You may receive a routine cleaning on your first appointment depending on your overall periodontal condition. 

This appointment is thorough and every aspect of this visit will be explained and discussed with you, the patient.
